We have a full-time vacancy for a Resource Officer within the Conquest Hospital Discharge Team, which is based on site at the Conquest Hospital. As a Resource Officer in Adult Social Care, you will play a key role in the assessment of adults who are currently in hospital and will support them to arrange care in order to facilitate their discharge. You will gain a detailed knowledge of the Care Act (2014), Mental Capacity Act (2005), Equality legislation and the Deprivation of Liberty safeguards, and will implement this in your daily practice. You will also be expected to carry out the Local Authorities safeguarding duties to protect and mitigate risks to the individuals we serve. We have excellent relationships with local partners and hospital trusts, and you will have the opportunity to work alongside other professionals to ensure good outcomes for the individual’s you are working with. Where necessary, we work with our local Integrated Commissioning Boards (ICB’s) to ensure people we serve get access to continuing care funding.
