Location: Otsego Office and Remote Hours: 8-10 hours/week with opportunity for growth Competitive hourly rate $37.00- $41.00 Benefits: generous health insurance reimbursement plan, 401k, paid time off, and on-demand pay What will I be doing? Participates in the review and oversight of nursing and home health aide services. Review patient admission assessments, care planning, goal setting, evaluation, coordination of care, emergency/safety plans, and discharge. Participates with Director of Home Health in training both new hire and existing qualified personnel. Ensure timely follow-up with patient hospitalizations, order supplies, and communicate with the interdisciplinary team. Perform accurate data entry in electronic medical record. Collaborate with outside providers and care coordinators to ensure smooth and reliable communication of patient’s plan of care. Maintain compliance with and stay abreast of changing county, state, and federal regulatory initiatives affecting home health care, including Medicare Conditions of Participation. Understand and comply with policy and procedures set forth by Accurate Home Care. Additional tasks, as assigned by manager. Qualifications Education Requirements: Must be licensed as a Registered Nurse (RN) in Minnesota. Additional Requirements: Maintain confidentiality in accordance with applicable laws and rules. One-year Medicare home health visit nurse experience preferred including proficiency with OASIS. Ability to read, write speak fluently in English. Confirmed negative tuberculosis status. Effective communication, interpersonal and critical thinking skills.
