RN Case Manager

This is a part-time (24 hours/week), primarily remote position for an experienced Registered Nurse passionate about delivering exceptional care and building meaningful relationships with clients and families. A day in the life of an RN Case Manager In this role, you will partner with clients, families, caregivers, and healthcare professionals to coordinate care and provide an exceptional client experience. This will be done by: Providing prospective clients with a warm and helpful introduction to Tribute. Managing the setup and day-to-day delivery of services for a caseload of clients. Communicating promptly, thoroughly, and proactively with all parties involved in each client's care. Providing back-up clinical support and supporting other Tribute markets as needed. This position is primarily remote and requires occasional travel to the Baltimore area approximately every 6–8 weeks, as needed.
