Education/Experience: Bachelor's Degree in a Human Services-related field preferred. This may be substituted with an Associates’ Degree and one-year of experience in working with people with Intellectual and Developmental Disabilities. Essential Duties and Responsibilities include the following: (Other duties may be assigned) Overseeing a caseload of up to 80 individuals. This includes: Processing all reimbursements for individuals, vendors, and staff; and ensuring that reimbursements meet all Medicaid and OPWDD requirements. Reviewing all staff notes on a monthly basis Monitoring expenses and reviewing monthly expenditure reports and sharing monthly reports to each individual’s team. Acting as the main point of contact for the individuals’ self-directed services. Attending biannual Life Plan meetings for all individuals and reviewing Life Plans to ensure all Medicaid and OPWDD requirements for service delivery and billing are met Reviewing Staff Action Plans created by Support Brokers to ensure they match the Life Plan Communicating with various Wildwood departments involved in a person’s Self-Direction services (HR, Finance, Training). Maintaining annual required OPWDD and Wildwood trainings Ensuring all documentation is scanned and saved for a period of 10 years, regular auditing of individual’s files, and maintaining organization of electronic files. Keeping up on OPWDD and Medicaid regulations as they pertain to Self-Direction services, and communicating these regulations to participants as needed. Compiling information for all individuals on their caseload who receive a Housing Subsidy; this may include doing Quality Assurance walkthroughs, home visits, assisting in gathering documents, and completing monthly progress notes for billing of services. Qualifications Additional Requirements: A valid NYS driver's license, daily access to a vehicle and driving record acceptable to the agency insurance carrier's standards are required.
