Job Title: Care Manager Job Description The Senior Care Manager assesses, plans, and implements comprehensive medical and supportive services for post-discharge members across the continuum of care, ensuring high-quality, cost-effective outcomes. This role focuses on complex care transitions, medication reconciliation, and collaborative care planning with interdisciplinary teams to support members’ transition and discharge needs. The Senior Care Manager provides education, resources, and coordination among healthcare and community providers to address both medical and socio-economic needs, while serving as a subject matter expert and contributing to continuous improvement of care management services. Responsibilities Assess members’ current health status, resource needs, services, and treatment plans, and provide appropriate clinical and non-clinical interventions. Plan and implement all aspects of medical and supporting services across the continuum of care for post-discharge members, promoting quality and cost-effective care. Complete pre-admission and post-discharge medication reconciliation, evaluating medications and ensuring accuracy between pre-admit and post-discharge regimens. Develop individualized care and service plans and collaborate closely with discharge planners, providers, specialists, and interdisciplinary teams to support member transition and discharge needs. Facilitate transition of members into care management services based on acuity level and identified needs. Provide or coordinate education and resource materials to members, authorized caregivers, and providers to promote wellness activities and improve overall quality of care. Coordinate services between primary care physicians, specialists, medical providers, and non-medical resources to address members’ medical and socio-economic needs. Conduct telephonic, digital, home, and/or other site outreach to assess member needs and collaborate with appropriate resources. Serve as a subject matter expert for staff on clinical and non-clinical questions or issues related to post-hospital outreach for members. Precept new hires as needed, supporting the development of core skills and providing guidance throughout the onboarding process. Collect, document, and maintain all member information and care management activities in accordance with state, federal, and third-party payer regulations. Provide feedback to leadership on opportunities to improve and enhance care quality and delivery in a cost-effective manner. Perform other related duties as assigned to support the overall effectiveness of the care management program. Essential Skills Strong medical and health care knowledge with the ability to assess complex clinical situations and care needs. Demonstrated experience in case management and care management, including coordination of services across the continuum of care. Proficiency in medication evaluation and reconciliation for pre-admission and post-discharge scenarios. Excellent customer service skills with the ability to communicate effectively and compassionately with members, caregivers, and providers. Ability to collaborate with interdisciplinary teams, including discharge planners, providers, specialists, and community resources. Competence in telephonic and digital outreach, with the capability to conduct assessments in remote and field settings. Strong documentation skills and attention to detail to ensure compliance with state, federal, and payer regulations. Requires a Master’s degree in Behavioral Health or Social Work. Requires 4–6 years of related experience in behavioral health, social work, care management, or a closely related field. Current licensure as LISW, LMFT, LMHC, LPC, or RN. Active RN license in Nebraska is required for candidates practicing as registered nurses. Additional Skills & Qualifications Ability to serve as a subject matter expert and resource for clinical and non-clinical questions related to post-hospital outreach. Experience precepting or mentoring new staff and supporting structured onboarding processes. Knowledge of wellness promotion strategies and resource navigation to improve member outcomes and quality of care. Familiarity with third-party payer requirements and regulatory standards affecting care management documentation and processes. Strong problem-solving and critical-thinking skills to identify opportunities for improving care quality and cost-effectiveness. Comfort working flexible schedules that include evening and weekend hours. Proficiency with digital communication tools and remote work technologies. Work Environment This is a remote position that allows you to perform care management and outreach activities from a home or off-site office environment using digital and telephonic technologies. The schedule requires availability during evenings on weekdays, with flexible hours between 2:00 p.m. and 8:00 p.m., and on weekends with flexible hours starting no earlier than 9:00 a.m. You will collaborate virtually with interdisciplinary teams and connect with members, caregivers, and providers through phone, digital platforms, and, when needed, home or other site visits. The work environment emphasizes professional communication, regulatory compliance, and effective use of remote tools to deliver high-quality, coordinated care. Job Type & Location This is a Contract position based out of Omaha, NE. Pay and Benefits The pay range for this position is $35.00 - $40.00/hr.
