Job title: Senior Care Specialist (Registered Nurse) – Work From Home Job Description: Seasonal Opportunity | Tentative Assignment End Date: June 2027 Sagility is seeking experienced and compassionate Registered Nurses (RNs) to join our team as Senior Care Specialists in a work-from-home capacity. This is a seasonal position with a tentative assignment end date of June 2027, subject to business and client needs. Employees who remain actively employed in the role through the required duration of the assignment may be eligible for bonuses and/or other incentives. Bonus and incentive programs, including eligibility requirements, performance expectations, payment amounts, and applicable terms, will be determined and communicated by Operations Leadership after joining Sagility. About the Role As a Senior Care Specialist, you will use your clinical expertise to support members with chronic and complex health conditions. Working by phone, you will assess members' needs, develop individualized care plans, provide education and support, and coordinate services to help members achieve their health goals. Using a Whole Person Care approach, you will work collaboratively with members, caregivers, healthcare providers, and multidisciplinary care teams to identify risks, address barriers to care, and promote improved health outcomes. What You Will Do Responsibilities may include: Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members. Develop actionable, individualized, and measurable care plans based on member needs and identified health risks. Establish health coaching relationships with members and provide education, encouragement, and support related to chronic condition management. Use clinical nursing judgment to assess member needs and coordinate appropriate interventions for acute situations. Support transitions of care and safe discharge planning, including coordination of durable medical equipment (DME), home health services, provider appointments, and other necessary resources. Help members and caregivers navigate healthcare benefits, services, and available care options. Assess physical, environmental, and psychosocial factors that may affect a member's health and overall well-being. Collaborate with multidisciplinary teams, including social workers, dietitians, pharmacists, providers, and other healthcare professionals. Promote member self-management by providing education and resources appropriate to individual health conditions and goals. Coordinate care and condition-management activities among members, caregivers, providers, and other members of the care team. Document member interactions, assessments, care plans, interventions, and outcomes accurately and in a timely manner within applicable systems. Required Qualifications To be considered for this opportunity, candidates must meet the following requirements: Active, unrestricted Registered Nurse (RN) license in the state of residence. Active Compact RN license permitting practice in applicable compact states. Minimum of 2 years of clinical nursing experience ; 3 or more years preferred. Strong clinical judgment, critical-thinking, and problem-solving skills. Ability to work independently and exercise appropriate nursing judgment in a remote environment. Intermediate to advanced computer proficiency , including Microsoft Word, Excel, and Outlook. Ability to efficiently navigate multiple computer systems and electronic medical records. Strong verbal and written communication skills. Preferred Experience Preference may be given to candidates with experience in: Caring for elderly and/or chronically ill populations. Care coordination, case management, or managed care. Medicare and/or Medicaid populations. Chronic condition management and patient education. Telephonic nursing, health coaching, or remote care management. Working as part of a multidisciplinary healthcare team. Compensation Starting pay: $37.00 per hour Starting pay may be negotiable based on relevant experience, qualifications, skills, and business requirements. Daily Pay: Eligible employees may have access to earned wages before their regularly scheduled payday, subject to applicable program terms.
