The successful candidate will support patients living with or at risk for HIV infection, including pregnant women living with HIV, through coordinated care, HIV prevention education, and linkage to appropriate clinical and community resources. Responsibilities include serving as a liaison between obstetric and infectious disease providers, collaborating with community partners, and supporting grant-related program activities. Responsibilities include serving as a liaison between obstetric and infectious disease providers, collaborating with community partners, and supporting grant-related program activities. Position is time limited. Position requires a level II background check. 55% – Clinical Care Coordination and Nursing Services • Coordinate and maintain clinical care of patients by reviewing and preparing charts for clinics. Triaging patient phone calls to determine priority of service, refilling medications as required, obtaining authorizations for medications/tests as required. • Collects, updates and maintains patient’s lab data, and patient information in client data base. • Attends and performs all clinical nursing functions at required clinics, assisting physician/ARNP with special exams, and medication administration as needed. • Assumes the role of Discharge Planner/Organizer and clinic coordinator during daily clinics. • Conduct weekly clinical team meetings. • Supervises daily clinical functions per policy and protocol. 10% – Community, School, and Agency Collaboration • Establish working relationships with referral sources. • Arrange and coordinate services with outside agencies for supplies and equipment. • Work in conjunction with Developmental Services, Child Protection Teams, child advocacy groups, support groups, and DCF. • Consult with school nurses, faculty counselors, and social workers assisting in the development of the individual Education Plan 10% – Care Coordination Planning and Interdisciplinary Collaboration • Collaborate with all members of the health care team to develop and implement individualized Client Care Coordination Plan; addressing changes in clients' needs and providing tools/resources along with other team member’s participation to assure for a comprehensive individualized, cost-effective care plan. 5% – Documentation, Reporting, and Compliance • Manage and maintain documentation records required for mandatory reports for sponsoring agencies and grant funding sources. Collaborate with Departmental Coordinator to assure for a comprehensive individualized, cost-effective care plan 4% – Information Systems and Technology Utilization • Maintain working knowledge of computer programs required to perform office activities, including but not limited to MS Word, MS Excel, MS Outlook and electronic medical records. 3% – Professional Development and Community Engagement • Engage in professional development activities by serving as consultant to other nurses and health professionals, attending professional conferences, workshops, meeting and supporting governmental and community agencies 3% – Confidentiality and Information Security • Adheres to DOH Information Security Policies, Protocols and Procedures to ensure the physical security and protection of confidential information are maintained. • Demonstrate an understanding of legal and ethical issues pertaining to confidentiality of client information and restrictions on the release of confidential information. 5% – Research and Special Assignments • Participates in research studies as assigned. Performs other professional nursing and related administrative duties as assigned. 5% – Administrative and General Office Support • Ensure appropriate communication with other staff members, completes other duties as assigned or necessary. • Answering office phones, copying materials, entry of lab data, tracking patient information, requests and pick up of medical records, purchase patient supplies. • Performs general office duties as needed. This position is located in St. Peterburg, Florida Minimum Qualifications: This position requires licensure as a registered nurse in accordance with Chapter 464, Florida Statutes, or eligibility to practice nursing in accordance with Chapter 64 B9, Florida Administrative Code and two years of nursing experience. Specific positions may require knowledge and experience in a specialized nursing area. Preferred Qualifications: • Experience in HIV care, prevention, and medical case management • Phlebotomy and injection administration experience • Experience with quality improvement, data tracking, or grant-supported clinical programs • Strong written and verbal communication skills, including patient education and public speaking • Proficiency in Microsoft 365 and Epic EMR
