Speech-Language Pathologist (SLP) – In-House Rehabilitation Providence Healthcare of Thomaston 1011 S. Green Street Thomaston, GA 30286 Employment Options Full-Time Part-Time PRN / Per Diem Flexible Scheduling Available Schedule Options Include: Traditional 5-day workweek Four 10-hour shifts (4x10) Three 12-hour shifts (3x12) Flexible start and end times Weekend-only opportunities Hybrid schedule with designated remote documentation and administrative time (based on operational needs, caseload, and performance) Job Overview Providence Healthcare of Thomaston is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our in-house rehabilitation team. We are hiring Full-Time, Part-Time, and PRN (Per Diem) Speech-Language Pathologists to provide high-quality therapy services within our skilled nursing and subacute rehabilitation setting. As a Speech-Language Pathologist, you will evaluate, diagnose, and treat residents with speech, language, cognitive-communication, voice, fluency, and swallowing disorders. You will collaborate closely with an interdisciplinary care team to develop individualized treatment plans that support functional recovery, safety, and improved quality of life following illness, injury, surgery, or hospitalization. Essential Duties and Responsibilities Perform comprehensive evaluations of speech, language, cognitive-communication, and swallowing function Develop and implement individualized treatment plans based on physician orders, clinical findings, and resident goals Provide evidence-based interventions for speech, language, cognition, voice, fluency, and dysphagia Assess swallowing function and recommend appropriate diet textures, strategies, and safety precautions Monitor resident progress and adjust treatment plans to maximize functional outcomes Complete accurate and timely documentation, including evaluations, treatment notes, progress reports, recertifications, and discharge summaries in accordance with Medicare, Medicaid, state, federal, and facility guidelines Collaborate with physicians, nursing staff, physical therapists, occupational therapists, dietitians, and other interdisciplinary team members to coordinate resident care Educate residents, families, and caregivers on communication strategies, swallowing safety, cognitive techniques, and home exercise programs Participate in interdisciplinary care planning meetings, family conferences, and discharge planning Maintain compliance with all professional, regulatory, and facility standards Support a resident-centered rehabilitation environment focused on quality outcomes and clinical excellence Qualifications Required Master's degree in Speech-Language Pathology from an accredited program Current Georgia Speech-Language Pathologist license in good standing Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred; Clinical Fellowship (CF) candidates may be considered Current CPR certification or willingness to obtain upon hire Strong clinical, communication, and documentation skills Ability to work effectively within an interdisciplinary healthcare team Preferred Experience in skilled nursing, subacute rehabilitation, long-term care, or inpatient rehabilitation Experience with dysphagia management and cognitive-communication disorders Familiarity with Medicare documentation and reimbursement guidelines Experience working with geriatric and medically complex populations Benefits Full-Time Employees Competitive compensation Health, dental, and vision insurance Paid Time Off (PTO) Paid holidays Retirement plan options Continuing education assistance Career advancement opportunities Supportive in-house rehabilitation team Part-Time & PRN Employees Competitive compensation Flexible scheduling Alternative schedules including 4x10 and 3x12 shifts Weekend-only opportunities available Continuing education opportunities Collaborative therapy team environment Why Join Providence Healthcare of Thomaston?
