City/State Norfolk, VA Work Shift First (Days) Overview: Overview Sentara Health is seeking a Supervisor, Integrated Care Management to provide leadership and operational oversight for a team of Integrated Care Managers supporting complex member populations. This role is responsible for supervising daily department operations, coordinating work activities, ensuring quality and service excellence, and supporting team performance through coaching, development, and competency assessment. The Supervisor serves as a first-line leader and works closely with care management staff to promote effective care coordination, member engagement, and achievement of clinical and operational goals. This position provides mentorship, education, and performance support to the team while partnering with organizational leaders to drive continuous improvement initiatives. The role also participates in case management activities as needed and supports telephonic assessments and interventions for Medicare and other complex populations. This is a remote position supporting members throughout Virginia . Key Responsibilities Supervise and coordinate the daily operations of the Integrated Care Management team Provide leadership, coaching, performance management, and professional development to direct reports Monitor quality, productivity, member outcomes, and compliance with organizational and regulatory requirements Support staff competency assessments, training initiatives, and ongoing educational activities Collaborate with interdisciplinary teams to ensure effective care coordination and resource utilization Participate in case management activities and provide direct support during periods of high volume or operational need Foster a culture of excellence, accountability, and member-centered care Education Bachelor's Degree in Nursing (BSN) required Master's Degree in Nursing, Social Work, Healthcare Administration, Public Health, or a related field preferred Certification/Licensure Active Registered Nurse (RN) license preferred Certified Case Manager (CCM), preferred For Integrated Care Management departments, specialty certification (ACM, CCM, CCTM, or RN-BC) required within one year of eligibility Licensed Mental Health Professional (LMHP) certification preferred Experience Minimum of three (3) years of case management experience required Previous supervisory, team lead, or leadership experience strongly preferred Experience managing complex populations, including Medicare members, preferred Experience conducting telephonic assessments, care planning, and care coordination interventions Knowledge of utilization management, population health, care transitions, and integrated care management principles Demonstrated ability to coach, mentor, and develop clinical staff while driving quality and performance outcomes Experience working within a health plan, managed care, healthcare system, or community-based care management environment preferred We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay rate for Full Time employment is:$87,443.20-$145,745.60. Additional compensation may be available for this role such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
