System Pharmacist (Full-Time) – Clinical: Internal Medicine/Transition of Care

Title: System Pharmacist – Clinical: Internal Medicine/Transition of Care Summary Details: This is a patient care–focused role with limited administrative duties, responsible for providing pharmacotherapy recommendations aligned with Atlantic Health System's strategic initiatives. This is a hybrid position, based on-site at the corporate office in Morristown with opportunities for remote patient care activities. The role supports multiple sites, providing pharmacist-specific pharmacotherapy services across the system. The pharmacist provides pharmaceutical care to patients across Atlantic Health inpatient settings, ensuring safe, effective, and evidence-based medication therapy. Key Responsibilities Review and verify medication orders for appropriateness, safety, and accuracy Monitor therapeutic outcomes and provide clinical recommendations to the healthcare team Collaborate with healthcare professionals across all sites to optimize patient care Participate in quality improvement and patient safety initiatives Ensure compliance with hospital, departmental, and regulatory policies Job Responsibilities / Essential Functions Reviewing patient medication regimens and providing recommendations to optimize therapy based on clinical guidelines and best practices Conducting medication reconciliation to ensure accuracy and safety during transitions of care. Collaborating with healthcare providers to develop and implement patient-specific care plans. Monitoring patient outcomes and adjusting therapy as necessary to achieve therapeutic goals. Contributing to quality improvement initiatives related to medication use and patient safety. Documenting interventions, clinical assessments, and recommendations in the electronic health record. Staying current with developments in pharmacotherapy and evidence-based medicine. Required qualifications: Graduated from accredited School of Pharmacy with PharmD PGY1 pharmacy residency training OR 2 years of clinical pharmacy experience in the assigned area Current active registration and licensure with the State of New Jersey Board of Pharmacy
