The clinician is responsible for assessing and identifying behavioral, educational, developmental and psychosocial functioning of the patient and completing a clinical assessment using DSM criteria to determine best patient care. The clinician is responsible for facilitating the exchange of clinical collateral with referral sources to coordinate transfers and admissions. The clinician is responsible for completing medical necessity reviews with the insurance companies both for precertification and concurrent review. Position Responsibilities: Clinical / Technical Skills (40% of performance review) Responds to initial notification within 15 minutes when receiving email notification of an assessment. Is prepared for tele-video assessment within 15 minutes of initial notification. Confirms appt with patient and is able to connect via Zoom to complete assessment. Completes the Intake Assessment within 45 minutes, preferably within 30 minutes. Initiates clinical collateral gathering of referral source material within 30 minutes of notification. Identifies need for the additional screenings to include: Substance Use Screening, Nutritional Screening, Abuse Screening and Functional Abilities Screening. For child and adolescent population, identifies needs for educational and developmental screens. Demonstrates knowledge of admission criteria for inpatient, partial hospitalization and intensive outpatient levels of care. Demonstrates knowledge of continued stay criteria for inpatient, partial hospitalization and intensive outpatient levels of care. Maintains a working knowledge of Medicare, Medicaid and Managed Care Organizations to assist with the most appropriate disposition and care for the patient. Demonstrates knowledge and competency in providing thorough and concise clinical information to the attending physician to secure treatment recommendation and disposition of the assessment. Communicate effectively with providers, intake staff, Outpatient staff and fellow coworkers via teams group chat or verbally via phone. Pre admits patients in Wellsky and sends message to appropriate Outpatient email group to notify them of patient who has been referred into their program. Demonstrates knowledge of level of care criteria such that can provide treatment recommendations based upon clinical presentation Ability to state local laws, ordinances, and practices governing involuntary hospitalization and ensure compliance with the same. Ability to state the procedure for managing medical and psychiatric emergencies according to EMTALA guidelines along with facility policies and procedures. Accurately documents all information related to a medical/psychiatric emergency transfer to include Memorandum of Transfers, consent/refusal for care, progress notes, MD consultation and signature. Demonstrate understanding of tele health schedule to include working required shifts per month and request for time off policies. Demonstrates an ability to be flexible, organized and function well in stressful situations. Performs other duties as assigned. Current state license in the state of employment in clinical social work, counseling license or per state of practice guidelines. Preferred: Current state license in the state of employment in clinical social work, or counseling license in states without required license regulations. Maintains education and development appropriate for position. Experience Required: Minimum two (2) years of supervised clinical experience. Preferred: Assessment and evaluation experience preferred. May substitute education for experience
