The Role

An exciting opportunity has arisen for an Advanced Practitioner to join the North East Rehabilitation Service. This is a developing role within the service, requiring a proactive, independent, and enthusiastic individual who enjoys building networks and developing new roles and care pathways.

You will work within the multidisciplinary team alongside the newly established Frailty Practitioner and will be expected to work across professional and geographical boundaries to provide comprehensive clinical care for patients within the Rehabilitation Service.

All roles will be assessed for sponsorship eligibility in line with current Home Office requirements. NHS Greater Glasgow and Clyde (NHSGGC) can only issue a Certificate of Sponsorship (CoS) where these are met.

Duration, Location, and Working Pattern:

  • This is a full-time post working 36 hours per week
  • The role is based within the North East Rehabilitation Service.
  • The service operates seven days a week; you will be expected to work across geographical boundaries as required.
  • A full driving licence and access to a vehicle are essential as you will be required to travel as part of your role.

Key Responsibilities

As an Advanced Practitioner, you will provide comprehensive clinical care to patients within the Rehabilitation Service. You will assess, diagnose, and manage the clinical needs of a diverse patient group, working both autonomously and collaboratively with members of the interdisciplinary team. You will manage your own caseload and have the authority to refer, admit, and discharge patients within appropriate clinical areas. You will play a key role in coordinating care and liaising with colleagues across acute services, primary care, and social work to ensure patients receive seamless, person-centred care. You will be responsible for ensuring the delivery of evidence-based practice, providing clinical leadership and professional advice to colleagues, and contributing to the development, implementation, and evaluation of standards of care across the service.

Knowledge, training, qualifications and/or experience required to do the job:

  • Registered Nurse or Allied Health Professional.
  • Significant post-registration experience within a healthcare setting, including experience at a senior practitioner level.
  • Master's level qualification in Advanced Practice, or currently working towards achieving this qualification.
  • Demonstrable ability to work independently and proactively.
  • Experience of multidisciplinary and integrated working across healthcare services.
  • Strong leadership, clinical decision-making, and care coordination skills
