Relavix Health Group is seeking an experienced clinical leader to join our team as a Regional Nurse / Clinical Services Director supporting our two Wisconsin skilled nursing facilities. This position will provide regional clinical leadership, guidance, and support to facility nursing leadership while promoting consistent clinical practices, regulatory compliance, quality outcomes, and a strong culture of resident-centered care. Job Summary The Regional Nurse / Clinical Services Director serves as a clinical resource and leadership partner for Relavix Health Group's Wisconsin facilities . This position works closely with facility Administrators, Directors of Nursing, MDS leadership, and interdisciplinary teams to strengthen clinical operations, develop nursing leaders, improve resident outcomes, and maintain compliance with federal and Wisconsin regulations. This is a highly visible, hands-on position that requires regular travel between the two Wisconsin facilities and the ability to identify opportunities, provide solutions, coach leaders, and follow through to measurable results. The Regional Nurse / Clinical Services Director will serve as a mentor, clinical resource, and accountability partner to facility nursing leadership while helping ensure that Relavix's clinical standards and best practices are consistently implemented across both facilities. Responsibilities Provide clinical oversight and leadership support to the nursing departments at both Wisconsin facilities, partnering closely with Administrators and Directors of Nursing to maintain high standards of resident care. Conduct regular facility visits, clinical rounds, and audits to identify risks, evaluate clinical performance, and ensure consistent implementation of Relavix Health Group's clinical standards and best practices. Coach, mentor, and develop Directors of Nursing and other clinical leaders while promoting accountability, strong communication, effective decision-making, and a positive nursing culture. Monitor clinical outcomes and quality indicators, including falls, pressure injuries, infections, rehospitalizations, weight loss, incidents, and medication concerns, and develop action plans for improvement. Support regulatory compliance and survey readiness by conducting audits, assisting with Plans of Correction, monitoring clinical documentation and policies, and serving as a clinical resource before, during, and after surveys. Provide clinical education and staff development by identifying training needs, supporting competency programs, and promoting evidence-based practices and continued professional development. Promote strong interdisciplinary collaboration among nursing, MDS, therapy, dietary, social services, activities, medical providers, and other departments to ensure coordinated, resident-centered care. Partner with the Vice President of Clinical Services and corporate leadership on clinical initiatives, policy development, quality improvement, leadership development, and special projects while providing regular updates on facility performance, opportunities, and clinical risks. Qualifications Current, unencumbered Registered Nurse (RN) license in the State of Wisconsin . Bachelor's degree in Nursing (BSN) preferred. Minimum of 5 years of nursing experience , preferably within skilled nursing/long-term care. Previous nursing leadership experience required; Director of Nursing, Assistant Director of Nursing, Regional Nurse, or similar leadership experience strongly preferred. Demonstrated knowledge of federal and Wisconsin skilled nursing regulations. Experience with state and federal surveys, clinical audits, QAPI, and Plans of Correction. Strong understanding of clinical documentation, quality measures, infection prevention, care planning, and resident safety. Experience coaching and developing nursing leaders. Excellent communication, organizational, critical-thinking, and problem-solving skills. Ability to work independently while collaborating effectively with facility and corporate leadership. Ability and willingness to travel regularly between Relavix Health Group's two Wisconsin facilities. Valid driver's license and reliable transportation.
