Job Duties: Job Description CommunityCare, a Federally Qualified Health Center Look-Alike affiliated with Geisinger and qualifies for the National Health Service Corp Loan Repayment Program and the Pennsylvania Loan Repayment Program . In this role, you will partner with interdisciplinary teams to manage population health, improve outcomes, and drive preventive care initiatives. The patient panel includes comprehensive care across the full lifespan, from newborns to end-of-life care.
