The Home First Response Service is a well-established multidisciplinary team committed to transforming care for frail older adults. An exciting opportunity has arisen for motivated and enthusiastic Advanced Frailty Practitioners to join the Home First Response Service (HFRS) within NHS Greater Glasgow and Clyde.
We are seeking clinicians with a passion for caring for older people and supporting the delivery of person, centred, frailty-focused care across the acute and community interface. The Home First Response Service works closely with Emergency Departments, Acute Assessment Areas, and Hospital at Home, community teams and wider multidisciplinary partners to prevent unnecessary hospital admissions, facilitate early discharge, and support patients to remain at home wherever possible.
The service plays a key role in improving outcomes for frail older adults through comprehensive assessment, clinical decision-making, care planning and coordination of services across health and social care settings.
All other roles will be assessed for sponsorship eligibility in line with current Home Office requirements. NHS Greater Glasgow and Clyde (NHSGGC) can only issue a Certificate of Sponsorship (CoS) where these are met.
Duration, Location, and Working Pattern:
- Location based in Queen Elizabeth University Hospital
- Permanent & Fixed-term Contract (6 months) Available
- Full-time positions, however various hours will be considered at interviews
- Shift pattern for this post will be 5 days over 7
Please note the salary is Pro Rata for part time hours.
Key Responsibilities
- Undertaking comprehensive frailty assessments for older adults.
- Supporting clinical decision-making and development of management plans.
- Facilitating safe discharge and admission avoidance where appropriate.
- Working collaboratively with acute, community and social care partners.
- Coordinating referrals and onward care arrangements.
- Providing leadership and expert clinical advice within the multidisciplinary team.
- Supporting service development and quality improvement initiatives.
Knowledge, training, qualifications and/or experience required to do the job:
Applicants should be registered healthcare professionals (e.g. Nurse, Allied Health Professional or Paramedic) with substantial post-registration experience, ideally within older people's services, urgent care, emergency care, acute medicine, Hospital at Home or community services.
You Should Demonstrate
- Excellent clinical assessment and decision-making skills.
- Experience of managing patients with frailty and complex needs.
- Ability to work autonomously and as part of a multidisciplinary team.
- Strong communication, leadership and organisational skills.
- A commitment to delivering high-quality, person-centred care.
Relevant postgraduate qualifications in either advanced clinical practice, frailty, or clinical assessment would be required.
