Key Responsibilities: Conduct psychosocial assessments and provide supportive counseling under supervision as required for LMSW licensure. Collaborate with the clinical team to develop and implement patient care plans. Provide case management services and connect patients with necessary community resources. Work closely with physicians, nurses, and other providers in a multi-specialty setting. Maintain accurate and timely documentation of patient interactions and services provided.
