Master's degree or doctoral degree, as applicable to the physician's professional credentials. Board certification in Internal Medicine, Family Medicine, or Emergency Medicine. Geriatric medicine experience, training, or certification is strongly preferred. Active, unrestricted physician license in the applicable state. Current state Controlled Dangerous Substances (CDS) certification, as applicable. Current DEA registration Eligible for or currently enrolled as a provider with Medicare and Medi-Cal Demonstrated knowledge of chronic disease management and geriatric medicine preferred. Ability to work collaboratively within an interdisciplinary care model. Willingness and ability to participate in a rotating night and weekend on-call schedule.
