Physical Therapist

Role: Physical Therapist providing personalized in-home, senior living, and virtual care to older adults.

Responsibilities

• Deliver one-on-one physical therapy in various settings, including private residences and senior communities.
• Conduct evaluations, develop individualized treatment plans, and provide evidence-based therapies to enhance mobility, independence, and quality of life.
• Document patient progress accurately within the EMR system.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Utilize telehealth and virtual care to extend access and support.
  
  

Qualifications & Skills

• Valid Physical Therapy license or eligibility.
• Degree from an accredited program.
• Proficiency in basic computer use and EMR systems.
• Current CPR certification.
  
  

Physical Requirements: Ability to exert moderate effort, assist with patient transfers, reposition patients, perform therapeutic activities, drive for home visits, and wear PPE when necessary.
