Role Overview: This position involves providing one-on-one physical therapy to older adults in their homes or senior living communities, using evidence-based treatments to enhance mobility, independence, and quality of life.
Responsibilities: Conduct evaluations, develop personalized care plans, deliver therapy sessions, document in EMR systems, collaborate with interdisciplinary teams, and educate patients and caregivers. Virtual care options may also be incorporated to increase flexibility and care access.
Qualifications & Skills: Valid PT license, degree from an accredited program, proficiency with EMR and basic computers, and current CPR certification. Physical capability to perform tasks such as lifting, repositioning, and assisting with transfers is essential.
