Role:
Licensed Physical Therapist providing personalized care to older adults in private homes and senior living communities, utilizing a flexible schedule and telehealth options.
Responsibilities:
Conduct evaluations, develop and implement individualized treatment plans, deliver evidence-based therapy to promote mobility and independence, document care in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and support aging in place through virtual and in-person sessions.
Qualifications & Skills:
Valid PT license, degree from an accredited program, proficiency with EMR and computers, current CPR certification, and ability to meet physical demands such as lifting and assisting patients.
