Physical Therapist – Outpatient Therapy

We are seeking a Physical Therapist to provide outpatient rehabilitation services in in-home settings, primarily for older adults. Responsibilities include evaluating patients, developing and implementing personalized therapy programs, and instructing patients and caregivers to promote independence. The role involves supervising support personnel, ensuring timely documentation, making multidisciplinary referrals, and maintaining a safe environment. Therapies offered include therapeutic exercise, manual therapy, modalities, functional training, and adaptive device fitting. The therapist will perform assessments, modify plans of care, and incorporate health and wellness initiatives. Qualifications include a valid state license, a relevant Master’s degree or equivalent experience, and reliable transportation.
