Role: Licensed Physical Therapist providing personalized, one-on-one care to older adults in private residences and senior living communities, utilizing the Geriatric House Calls™ model.
Responsibilities: Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy to improve mobility, independence, and overall wellness.
Document care within EMR systems, collaborate with interdisciplinary teams, and educate patients, caregivers, and staff to promote safety and aging-in-place strategies.
Incorporate telehealth and virtual care to enhance service flexibility and continuity.
Qualifications & Skills: Valid state PT license or eligibility, degree from an accredited program, basic computer and EMR proficiency, and current CPR certification.
Must meet physical demands of in-home care, including lifting, repositioning, and driving to patient locations.
