We are seeking a licensed Physical Therapist to provide one-on-one care to older adults in private residences and senior living communities.
Responsibilities include delivering therapy to enhance mobility, independence, and quality of life, documenting patient progress in our EMR system, and collaborating with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
The role offers autonomy, flexibility, and the opportunity to incorporate telehealth and virtual care to extend access and support more patients.
Qualifications include a valid PT license in the practice state, degree from an accredited program, and basic computer and EMR skills. Candidates must be able to meet physical demands such as patient transfers and driving to care sites.
