Job Title: Primary Care Provider
Salary Range: $72.12–$91.35/hour + Annual Bonus
Location: Fresno, CA
Shift: Rotating night and weekend call

Requirements to Apply
• Graduation from an accredited School of Nursing or Physician Assistant studies
• Current California NP or PA license with prescriptive authority
• Minimum 2 years of NP/PA experience
• Minimum 2 years of experience working with seniors; geriatric experience preferred
Primary Care Provider role supporting seniors through comprehensive assessments, primary care, chronic care management, and interdisciplinary coordination.
Provide comprehensive primary care, assessments, care coordination, and interdisciplinary leadership while improving outcomes for vulnerable senior participants in community settings.
