Are you passionate about supporting older people to maintain their independence, wellbeing and quality of life? Join the NSW Health Aged Care Assessment – Hunter team and play a key role in connecting older people with the right supports to meet their individual needs.

NSW Health Aged Care Assessment – Hunter is a large, supportive team providing services across the Hunter region, including Newcastle, Lake Macquarie, Port Stephens and the Lower Hunter. With our main office located in Wallsend, our team works across community, residential aged care and hospital settings to deliver high-quality assessment services for older people.

About The Role

As a Non-Clinical Assessor, you will complete home support assessments for older people under the Aged Care Act 2024, using the Integrated Assessment Tool (IAT) to identify appropriate supports for people with entry-level care needs.

You will conduct comprehensive assessments that consider an individual’s environment, physical functioning, independence, social circumstances and existing support networks. Following each assessment, you will develop an individualised Support Plan that outlines assessment outcomes, personal goals and recommended services to support the person’s wellbeing and independence.

This role requires the ability to work autonomously within the community while collaborating with a multidisciplinary team to achieve positive outcomes for older people.

About You

The successful candidate will have:

  • Qualifications and experience working within community aged care.
  • Experience and skills in completing community-based aged care assessments.
  • Demonstrated ability to assess individual needs and identify appropriate services using the principles of wellness and reablement.
  • A strong interest and experience in supporting older people within the community.
  • Excellent time management skills, with the ability to prioritise competing demands.
  • The ability to work independently while contributing effectively within a multidisciplinary team.
  • Strong computer skills, communication skills and documentation capabilities, with confidence working in a mobile computing environment.

What You'll Be Doing

Provide basic home based face to face assessments of clients referred via the My Aged Care Gateway using the National Screening and Assessment Form (NSAF). Working with each client, the Home Support Assessor establishes goals specific to the client; develops appropriate strategies and actions, including referral to aged care and non –aged care specific services; and develops a Support Plan. The Home Support Assessor completes the NSAF and matches and refers clients to appropriate services.
