What You’ll Do: Conduct nutrition assessments, consultations, and follow-up evaluations. Provide individualized nutrition care plans and 1:1 resident consultations. Manage nutrition care for residents with dialysis, tube feeding, diabetes, cardiac conditions, wound healing, weight changes, and other complex clinical needs. Develop and implement individualized nutrition care plans. Monitor nutritional status and recommend appropriate interventions. Collaborate with healthcare professionals to improve nutrition outcomes. Utilize the Nutrition Care Process to deliver high-quality nutrition care. Make a meaningful impact by using Food as Medicine to improve residents' quality of life.
