Provide clinical recommendations to physicians and healthcare staff regarding medication therapy changes and optimizations. Maintain up-to-date patient/resident profiles, including medical history, lab results, and current medications (prescription, OTC, and home remedies). Ensure laboratory tests required for monitoring high-risk medications are appropriately ordered, reviewed, and communicated to the healthcare team. Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support. Document all communications and clinical interventions in the patient’s medical record, including interactions with families and caregivers. Implement and enforce policies and procedures for the safe handling, dispensing, and clinical oversight/management of medications. Ensure compliance with all state and federal regulatory requirements related to pharmacy practice and medication management. Report medication-related errors and adverse drug events to the Pharmacy Manager as required. Educate and support clinical team and healthcare agency staff on safe and effective medication handling procedures. Perform other pharmacy-related tasks and responsibilities as assigned. Qualifications Qualifications Required: Bachelor’s Degree in Pharmacy, Pharm D preferred One year experience in long term care desired License/Certification(s): current and valid pharmacist license in the state of practice Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA). Additional Job Information Key Facts Schedule: 7 days on / 7 days off (evenings or nights preferred based on coverage needs) Location: Fully Remote Setting: Critical Access Hospitals (CAH) + Behavioral Health Licensure Required: Active Pharmacist License – Texas ( required ) Active Pharmacist License – New Mexico ( required ) Willingness to obtain additional state licenses ( required ) Experience Preferred: Inpatient hospital pharmacy, remote verification, or clinical pharmacist role Core Responsibilities (Operational Workflow) 1) Order Verification (Primary Function) Review and verify medication orders in real-time Ensure: Appropriate indication Correct dose, route, frequency Renal/hepatic adjustments Allergy and interaction screening Release verified orders to ADCs (Omnicell/Pyxis) for nursing access 2) Clinical Review & Intervention Perform prospective and retrospective clinical order review Provide recommendations for: Antibiotic stewardship (IDSA-aligned) Renal dosing adjustments High-risk medications (anticoagulants, opioids, insulin, vancomycin AUC) Communicate directly with providers and nursing staff 3) Remote Oversight of Medication Use Monitor medication administration trends and discrepancies Support nursing workflow: Timing of doses Missing medication resolution Provide guidance on IV compatibility, infusion rates, and administration safety 4) Controlled Substance & Compliance Oversight Review narcotic usage patterns and discrepancies Support diversion monitoring efforts (DEA expectations) Ensure: Valid order present for all controlled substance dispensing Documentation aligns with administration records 5) Regulatory & Survey Readiness Maintain compliance with: CMS Conditions of Participation (CoPs) Joint Commission Medication Management (MM standards) Texas State Board of Pharmacy (TSBP) Support documentation and audit readiness (72-hour retrieval standard) Regulatory Requirements vs Best Practice Regulatory (Must Do) Active licensure in all practicing states (TSBP, NM Board) Prospective drug utilization review (DUR) prior to dispensing (CMS CoPs) Controlled substance oversight (DEA compliance) Timely documentation and retrievability of pharmacy actions Best Practice Antibiotic stewardship interventions (IDSA guidance) AUC-based vancomycin monitoring when appropriate Standardized remote verification workflows Clear communication loops with nursing and providers Operational Recommendations Experience with: Remote access platforms (VPN/Parallels) ADC systems (Omnicell, Pyxis) Meditech, Cerner, CPSI / TruBridge or similar EHRs Ability to work independently with strong clinical judgment High attention to detail under low supervision environments High-Risk Areas (Patient Safety + Compliance) Anticoagulants (heparin, DOACs) → bleeding risk, dosing errors Opioids (Schedule II) → diversion + respiratory depression Insulin → hypoglycemia risk Electrolytes (K, Mg, Na) → cardiac/neurologic complications Vancomycin / Aminoglycosides → nephrotoxicity Required Qualifications PharmD or BS Pharmacy degree Active licenses: Texas (required) New Mexico (required) Willingness to obtain additional state licenses Minimum 2+ years inpatient hospital experience preferred Strong clinical knowledge and independent decision-making ability Preferred Qualifications Experience in remote pharmacy services Familiarity with critical access hospitals (CAH) Experience with: Antibiotic stewardship programs Pharmacokinetics (vancomycin AUC dosing) Remote technician oversight Compensation & Benefits Competitive hourly rate (based on experience) Additional compensation for multi-state licensure PTO structure based on 7/7 schedule model Opportunity to grow with expanding hospital network Why This Role Matters This position plays a critical role in: Ensuring safe medication use across multiple hospitals Supporting limited onsite staffing environments Maintaining regulatory compliance and survey readiness Enhancing clinical outcomes through remote expertise
