In this role, you'll work closely with patients requiring anticoagulation management, helping improve outcomes through education, monitoring, care coordination, and evidence-based clinical support. The role involves assessing, monitoring, and adjusting anticoagulation therapy, educating patients, and coordinating care to maintain safety and therapeutic goals. Additionally, the anticoagulation Care Coordinator will work with physicians and other patient care providers as he/she supports the PCMH initiatives in meeting quality process and performance objectives through data collection, data analysis and process outcome evaluation. Experience A minimum of five years of relevant clinical experience. Demonstrate leadership and autonomy in nursing practice. Strong understanding of anticoagulation and lab interpretation. Education Licensed Registered Nurse required. Bachelor’s degree in nursing or related field preferred. Licenses The applicant must have a current license as a Professional Registered Nurse in their state of practice prior to the position’s start date. Additional state licensure must be obtained within 6 months of hire. Patient outreach and contact will be limited to those patients living in the state of current licensure until dual licensure is obtained. Essential Functions Clinical Knowledge & Assessment Understand coagulation processes, vitamin K role, thrombogenesis, hypercoagulable states, and antithrombotic therapy Identify cardiovascular indications for anticoagulation and explain mechanisms of warfarin and other anticoagulants Assess impact of medications, disease states, diet, lifestyle, and psychosocial factors on therapy adherence. Patient Education & Management Provide initial and ongoing education on anticoagulants, diet, activity, and self-testing Manage pre- and post-procedure warfarin protocols; develop Lovenox bridging schedules . Instruct patients on subcutaneous anticoagulant administration (e.g., Lovenox) Lab Monitoring & Dosing Adjust warfarin dosing based on INR results, clinical judgment, and department protocol. Monitor for signs of bleeding or thromboembolism and refer appropriately Coordination & Continuity of Care Communicate with other anticoagulation nurses, department medical director, providers, pharmacists, and physicians Develop a patient specific care plan with associated goals. Assess and address any barriers to care and goal attainment. Supports the PCMH initiatives at the practice level through involvement with quality improvement initiatives, participation in PCMH/Population Health meetings, and educating on best practice models as needed. Is an active member of the Care Coordination team through participation in regular team meetings and regional Case Management meetings, as well as leading discussions and teachings to other members of the care coordination team on their area of expertise. Engages in ongoing professional development.
