We are looking for Social Workers to support us to deliver our vision of supporting people to achieve what matters to them, living independently in a place they call home. Joining our Older People and Physical Disabilities (Community neighbourhood teams) as a (insert job role) in this role you will support adults to live well, safely and as independently as possible in their own homes and communities. You will work in a strengths-based way with people, carers and partners to understand what matters most and to build support around their needs. This role will be based in Chorley and covers South Ribble East. This role is hybrid working with office attendance twice a week (Minimum).
