This is an exciting opportunity to work as part of our Transition to Adult Social Care service, supporting young people as they move from Children’s to Adult services. This is a unique role where your social work values, professional curiosity and relationship-based practice are key to achieving positive, long-term outcomes.

  • Location: Mendip and South Somerset
  • Starting salary £37,280 rising annually, currently up to £41,777
  • Full time, 37 hours per week.

Role Purpose

As a Social Worker within the Transition to Adult Social Care team, you will support young people and their families as they prepare for adulthood, ensuring a smooth and person-centred transition into adult services.

You will undertake Care Act assessments, reviews, mental capacity assessments, safeguarding enquiries and Court of Protection work, supporting individuals to live as independently as possible.

This role offers the opportunity to build meaningful relationships with young people, focusing on strengths-based approaches and enabling positive, long-term outcomes as they move into adult life.

Key Responsibilities

  • Undertake strengths-based Care Act assessments, reviews and support planning, including risk assessment
  • Complete Mental Capacity Assessments, Safeguarding Enquiries and Court of Protection work
  • Manage a varied caseload, ensuring plans are personalised, outcome focused, proportionate and reviewed regularly
  • Work directly with young people and families, promoting independence and informed decision-making
  • Undertake carers assessments in line with statutory responsibilities
  • Respond to referrals and urgent situations as part of a Triage system, including crisis intervention
  • Prepare clear, professional assessments and reports to inform decision-making
  • Work collaboratively with partner agencies to deliver coordinated, person-centred support
  • Contribute to service decision-making, including care planning and resource allocation
  • Maintain accurate and timely records in line with legislation and policy

What We’re Looking For – Essential Skills/Experience

We’re looking for social workers who are committed to person-centred, strengths-based practice and who are passionate about supporting young people transitioning to adult social care services.

You Will Have

  • A recognised Social Work qualification and current registration with Social Work England
  • Experience or strong interest in working with adults
  • Ability to manage risk and make sound professional decisions
  • Strong communication skills, including working with individuals with a range of communication needs
  • Ability to work collaboratively across agencies while maintaining a clear social work perspective
  • Commitment to promoting independence, dignity and inclusion

About The Team/Service

This is an exciting opportunity to join the Transition to Adult Social Care Team and be part of the ongoing My Life My Futures programme; developing your practice within a team that values reflection, learning and development.

You will join a busy, supportive and experienced Transition team, which sits within the wider Learning Disabilities service working alongside Social Workers and Adult Social Care Practitioners.

You will work within a collaborative, multi-agency environment, building strong relationships with Children’s Social Care, the SEND Team, Education partners, health colleagues and local services

Additional Information

Please Note: We may consider offering Skilled Worker visa sponsorship to suitably qualified overseas social workers who are already residing in the UK. To be eligible, applicants must hold a recognised social work qualification and be registered with Social Work England. Sponsorship will be assessed as part of the recruitment process. This is only available to candidates currently living in the UK, as unfortunately we are unable to provide resettlement support.

Due to the nature of the role and the geographical area of the service across Somerset, which is a largely rural county, with limitations around public transport the ability to be able to travel to any location is essential. Therefore, applicants must have full access to a vehicle that can be used for work purposes.

Knowledge and understanding of Social Care practice as defined by Social Work England Guidelines, appropriate legislation and issues relevant to client group.

Ability to speak fluent English as stated in Part 7 of the Immigration Act (2016).

Understanding of social care resources and provision available beyond statutory agencies.

X

Awareness of government guidance and legislative changes across broad range of Social Service issues.

X

Relevant multi-disciplinary Social Care experience within multi-disciplinary team - relevant to needs of post.

X

Evidence of partnership working with other agencies including Social Care Services, Health Services, Voluntary Agencies and/or Education.

X

A variety of experiences working with relevant client group including Learning Disabilities.

X

Supervisory experience.

X

Previous experience of working with health professionals.

Possession of recognised Social Work Qualification – Degree in Social Work (BA), a master’s degree in social work, or equivalent.
