POSITION SUMMARY Relocation Assistance available up to $15,000 $135,720/yr. to $220,542.40/yr. The Senior Director, Value-Based Primary Care and Provider Partnerships is a strategic Ambulatory leader responsible for driving enterprise-wide transformation across the ambulatory care continuum. This role integrates population health strategies with operational excellence to advance the quintuple aim: improving quality, enhancing patient outcomes, reducing costs, and increasing value. Monitors performance and strives for excellence through ongoing process improvement. Contributes to a positive work environment that promotes HMC values. ESSENTIAL FUNCTIONS Job Duties & Responsibilities Function Management. Directs and coordinates the activities of assigned departments/functions. Functional management duties and responsibilities include, but are not limited to: Strategic Transformation & System Leadership: Partner with executive leadership to design and execute multi-year transformation roadmaps. Serve as the primary subject matter expert on market evolution, representing the organization within the OHSU Health Clinically Integrated Network (CIN) and clinical governance committees to ensure alignment across the health system. Value-Based Strategy & Performance: Own the strategy and financial outcomes for all risk-based contracts (MA, MSSP, Commercial). Operationalize complex contract terms into standard clinical workflows, aligning incentives, patient attribution, and risk adjustment to optimize the organization’s market position and ROI. Ambulatory Operations & Care Delivery Innovation: Oversee business operations across ambulatory settings using Lean/Six Sigma and Tiered Daily Management. Redesign care models (virtual/hybrid, team-based) and deploy enabling technologies like AI and Remote Patient Monitoring (RPM) to increase throughput and reduce operational waste. Population Health & Health Equity: Design outreach and care models for high-risk populations by integrating Social Determinants of Health (SDoH) into stratification. Set and monitor disparity-reduction targets, embedding culturally responsive practices and community partnerships to advance health equity and gap closure. Provider Alignment & Partnership Development: Serve as the executive liaison for employed and affiliated providers to cultivate trust and bidirectional communication. Co-develop performance scorecards, streamline community referral management, and address clinician well-being to ensure a sustainable, high-engagement workplace culture. Quality, Safety & Evidence-Based Practice: Standardize evidence-based care pathways and gap-closure campaigns to meet national quality metrics and reduce readmissions. Benchmark high-performing national programs to identify and adapt clinical innovations, ensuring all programs meet regulatory and compliance standards. Foster a culture of curiosity and psychological safety where frontline staff are empowered to pilot new ideas and scale successful outcomes. People Management. Provides leadership oversight and direct supervision for assigned staff to support high quality care and services, enhance patient/customer satisfaction, and drive results. Executes leadership and supervision tasks in an accurate, equitable, timely and effective manner to meet service and performance expectations. People management duties and responsibilities include, but are not limited to: Directly leads and coaches a high-performing team, fostering a culture of professional growth and continuous learning. Build a high performance, psychologically safe culture centered on access, safety, equity, experience, and accountability. Ensure consistent application of policies, bargaining unit agreements, performance management, and recognition programs across multiple sites. Communicates organizational goals and engages staff in developing departmental objectives and workplans. Communicates status updates transparently and consistently. Manages and leads employees from selection and hiring through offboarding/separation. Coaches, mentors, and supports managers and subordinate staff in the performance of their duties. Establishes clear expectations and fosters an environment that supports good communication through effective use of formal and informal discussion, 1:1’s, huddles, and staff meetings. Cultivates a high-engagement workplace culture, evidenced by measurable improvements in staff retention, safety, and leadership effectiveness metrics. Proactively monitors and manages employee satisfaction and engagement, turnover, training and development, and performance. Responds to union grievances. Takes appropriate action to identify and resolve concerns or issues. Mediates conflicts between staff and/or patients and staff. Develops and implements skills-training and scripting for dealing with difficult patients/customers and service recovery appropriate to the setting. Designs and implements a nursing leadership pipeline through active mentorship, intentional stretch assignments, and robust professional development. Plans and oversees department staffing and productivity. Financial Management: Leads department financial processes and related activities. Financial management duties and responsibilities include, but are not limited to: Develop and manage operating and capital budgets; aggressively mitigate variances; steward resources to support system priorities. Identify cost savings, revenue enhancement, and productivity opportunities; partner with Revenue Cycle to optimize reimbursement, reduce denials, and manage claims. Build programs to monitor, analyze, distribute, and act on clinical/operational/financial data; maintain transparent KPI reporting and executive communication. Responsible for decisions within scope of position and personal expertise. Addresses complex issues, applies expertise and sound judgement to approve deviations from standard procedure; interprets and/or develops new policies and procedures. Escalates issues to executive team when appropriate. Coordinates with peers and executive leaders to plan and develop operational and capital budgets that are aligned across the organization. Acts as the decision maker for assigned departments, within the scope of approved budgets and operational plans. Analyzes data and uses sound judgement in making independent decisions. Strategic Leadership: Participates in strategic planning for the organization Develops and leads implementation of departmental strategies and work plans. Provides input for organization-wide plans; serves as a subject matter expert and thought-partner. Promotes awareness of strategic initiatives, goals and objectives through regular communication and education. Inspires commitment and leads others to implement and execute plans. JOB SPECIFICATIONS Education & Experience REQUIRED: Master’s degree in Healthcare or Business Administration or a related healthcare field. Seven (7) years of progressing healthcare leadership experience with four (4) or more years’ experience providing leadership oversight and direct supervision of assigned staff. Five (5) years of experience in a physician managed care, value-based contracting, or CIN/ACO leadership position with demonstrated competencies. Required Knowledge, Skills, and Abilities Knowledge of: Ambulatory & Clinical Operations: Comprehensive understanding of clinic business operations, workflow optimization, and hospital-based ambulatory regulatory and accreditation standards. Value-Based Care (VBC) Mechanics: Deep insight into risk stratification, quality measures, incentive structures, utilization management, and PMPM economics. Payer & Market Dynamics: Understanding of Medicaid and commercial managed care organizations, payer requirements, and reimbursement methodologies System Governance: Knowledge of Clinically Integrated Network (CIN) design, physician alignment strategies, and network performance frameworks. Financial & Project Principles: Knowledge of financial modeling, cost/utilization trends, contract impact analysis, and project management methodologies. Skill in: Advanced Analytics: Proficiency in interpreting clinical, financial, and operational data to synthesize actionable insights and executive-level recommendations. Project Execution: Strong skill in planning, executing, monitoring, and sustaining complex initiatives across multiple departments. Strategic Communication: Excellent interpersonal and organizational skills, with the specific skill of conveying complex information to varied audiences (providers, executives, and staff). Conflict & Problem Resolution: Skilled in navigating sensitive operational or interpersonal issues through conflict resolution and creative problem-solving. Technical Tools: Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and specialized healthcare analytics tools. Relational Collaboration: Building and maintaining relationships to align multidisciplinary teams around quality, utilization, and cost performance drivers. Ability to: Navigate Ambiguity & Complexity: Leading organizational change with the resilience and adaptability required to handle complex, shifting environments. Executive Delivery: Ability to deliver on enterprise goals through effective leadership, prioritization, and rigorous follow-through. Multitasking & Complexity Management: Ability to manage multiple large-scale projects while simultaneously overseeing the daily operational needs of the division. Influence & Credibility: Ability to build trust, credibility, and sustained engagement among physicians, advanced practice providers, and operational leaders. Strategic Integration: Ability to align and integrate disparate clinical, operational, and financial levers to improve quality, equity, and cost outcomes. Constructive Collaboration: Ability to work positively and constructively with staff and providers from diverse backgrounds and clinical specialties.
