Band 6 Triage Clinician to join our
West Essex Care Coordination Centre (CCC).
About The Role
As a Triage Clinician, you will play a pivotal role in ensuring patients receive the right care, in the right place, at the right time. Working within a fast-paced, supportive environment, you will:
- Triage incoming referrals with a strong focus on therapy pathways
- Support both planned and unplanned care coordination
- Provide expert clinical decision-making and prioritisation
- Work collaboratively with multidisciplinary teams across health and social care

Main duties of the job
The post holder will be responsible for triaging all referrals coming into the West Essex CCC service across Unplanned care, Integrated Community Care Team (ICCT), and Transfer of Care Hub (TOCH), with the primary aim of strengthening existing triage team, improving patient flow, and supporting neighbourhood working. The role will support the current band 7 triage coordinator.
The post holder will be responsible for communicating with various acute hospitals, community teams, neighbourhood teams including primary care and external parties.
Provide therapy-led clinical decision-making to avoid unnecessary admissions, facilitate early discharge, support same-day responses and select appropriate pathways for community based referrals for the integrated neighbourhood teams.
