Within the General and Specialist Medicine Care group is Health Care of the Older People and Acute Frailty. The post holder will be a part of our Acute Frailty team, ensuring that the highest standards of care, in accordance with the National Service Framework for Frailty and relevant NICE guidance are provided to patients.

There is good multidisciplinary liaison and patient management with regular multidisciplinary conferences/ward rounds on the rehabilitation wards. These include nursing, physiotherapy, occupational therapy, social work, chiropody, speech therapy and dietetics. A good voluntary sector and League of Friends support the hospital.

We work closely with the department of Health Care of the Older People (HCOOP), who manage a number of our inpatient beds as well as support the front door activity and acute frailty.

The appointee will be responsible for caring for older people within acute frailty and expected to play an active role in participating with multi -disciplinary teams and other medical specialties to deliver the appropriate care for patients.

It is expected that the post holder will play an active part in the training of the Frailty Acute Clinical Practitioner’s. He/she will be expected to supervise, educate and assist them with their education and studies.

The post holder should take an active part in clinical audit, journal club and departmental teaching sessions. Regular appraisals will occur and the post holder will be expected to take part in CPD.

The appointee will also ensure that records of patients in your care are accurate, up to date, legible and signed.
