Join AmeriCorps as a Health Educator with a focus in Health Promotions through Lending Hands for Life at Erie Family Health Centers. This full-time service year opportunity supports health education, chronic disease prevention, patient engagement, and HIV-focused outreach through the Lending Hands for Life program. Members gain hands-on experience in community health while making a meaningful impact on underserved communities. The work schedule for this position is aligned with Erie's hours of operations. Main Duties & Responsibilities Provide one-on-one health education and self-management support to patients Participate in community events, training, and AmeriCorps service activities Support Lending Hands for Life HIV programming and re-engagement efforts Conduct outreach and engagement for patients requiring additional support Document patient interactions and coordinate follow-up care activities Facilitate health education groups and recruit participants for programs **Applications must be submitted by September 13, 2026, to be considered for full-time opportunities. Anticipated start of Monday, September 21st 2026. Qualifications Education Required : High school diploma or equivalent. Skills and Knowledge Required: Strong verbal and written communication skills. Ability to work with diverse populations. Current CPR certification U.S. citizen, national, or lawful permanent resident. Ability to meet AmeriCorps background check requirements. Preferred: Experience in community health, social services, or related fields. Intermediate Microsoft Office skills. Experience with electronic health record systems. Bilingual English/Spanish skills.
