Bilingual Care Navigator (English/Spanish), Remote

Care Continuity is hiring a Bilingual Care Navigator , fluent in English and Spanish, to help patients get the follow-up care they need after leaving the hospital. You'll contact recently discharged patients and help them schedule their recommended follow-up appointments. For patients who accept our services, you'll call their physicians' offices to book those appointments. Your work helps patients make a smooth, healthy transition from hospital to home. This role suits someone who enjoys working independently on daily assignments, pitches in to support the team, and cares about patients. What You'll Do Make and receive calls with patients and provider offices in English and Spanish Schedule follow-up appointments on patients' behalf, based on their preferences and availability Review patient charts (written in English) to understand follow-up recommendations Verify insurance coverage using online tools and resources Send and receive medical records by fax Write clear, detailed notes in English on patient scheduling preferences and call outcomes Manage your daily workload independently and support teammates when needed What You'll Need Fluency in English and Spanish , both spoken and written High school diploma or equivalent At least 18 years old Strong customer service and communication skills Comfort working across multiple windows and computer applications at once Ability to troubleshoot basic computer issues on your own A quiet, distraction-free home workspace and reliable high-speed internet A self-motivated, dependable, and organized work style, with strong multitasking skills Nice to Have At least one year of college completed Experience in healthcare, a medical office, a call center, or customer service Familiarity with medical terminology or health insurance Who Thrives Here Our best Care Navigators put people first.
