FedPoint is seeking a Care Coordination Case Manager to join our FLTCIP Care Coordination team.
As a Care Coordination Case Manager, you will deliver detailed, non-clinical support in the coordination of care for long-term care insurance claimants. You’ll educate claimants and their representatives on program requirements, provide clear information, and offer appropriate referrals. This role interacts directly with insured individuals, their legal representatives, and service providers to ensure all communication and issue resolution is accurate, timely, and responsive.
Key Responsibilities:
- Manage a caseload of claimants, serving as their primary point of contact
- Review medical records, handle transition cases, and update Plans of Care
- Provide explanations of insurance plans and benefits
- Respond to telephone and written inquiries regarding care coordination
- Advise management of critical issues and participate in risk management reviews
- Uphold compliance and fraud management principles
Qualifications:
- High school diploma or GED required
- 4+ years’ experience with long-term care insurance or other insurance operations in a support role
- LPN, LNA, or CNA certification or medical field experience preferred
- Experience with case management and/or claims preferred
- Strong customer service and communication skills
- Proficient in Microsoft Applications
- Ability to build strong working relationships and work well in a team environment
What to Expect on Pay:
The salary range for this role is $$22.59 – $30.56 per hour. We’ve shared the full range to stay transparent — but most new hires join us between the minimum and the midpoint, depending on skills and experience.
Final compensation is based on a mix of factors including qualifications, experience, and education, as well as internal equity and budget considerations.
Schedule: 40 hours per week Monday- Friday
Location: Portsmouth, NH Hybrid - 2 days per week on site
