How You’ll Contribute As a Care Coordinator, you help patients navigate their care with confidence while supporting the day-to-day operations that make high-quality virtual care possible. What you’ll do Serve as a warm, reliable point of contact for patients, helping with scheduling, questions, and follow-up needs. Coordinate appointments, confirmations, documentation, and patient communications with accuracy and care. Support insurance verification, pre-authorization workflows, and other administrative steps that keep care moving smoothly. Partner closely with clinical and operations teams to support consistent, compliant, patient-centered care. Help patients stay engaged in their care plans through proactive, empathetic outreach. What we’re looking for 2–5 years of experience in healthcare coordination, patient support, medical office administration, referral coordination, insurance verification, prior authorization, or a similar patient-facing healthcare operations role. A strong ability to build trust with patients, communicate clearly and compassionately, and help patients navigate next steps in a virtual care setting. Experience supporting scheduling, documentation, patient outreach, insurance workflows, referrals, or other care coordination activities that keep care moving smoothly. Strong organization, follow-through, and attention to detail, with comfort managing multiple patient touchpoints, competing priorities, and time-sensitive administrative tasks. Able to efficiently use and adapt to EHRs, telehealth platforms, and a variety of digital communication and workflow tools. Interest or experience working with Medicare, Medicaid, FQHC, community health, underserved, or complex patient populations preferred.
