Health Coach, Virtual Obesity Medicine

How You’ll Contribute As a Health Coach, you’ll support patients in our virtual obesity program through ongoing coaching, education, and care coordination. You’ll play a key role in driving engagement, behavior change, and overall patient experience - working closely with providers and the care team. What You’ll Do Conduct regular virtual check-ins to track progress and motivate patients Support lifestyle, nutrition, and behavior change goals Serve as a primary point of contact for patient questions and support Coordinate with providers, dietitians, and care team members Document patient interactions in the EHR Escalate clinical concerns to licensed providers as needed What We’re Looking For 2-5 years of experience in health coaching, patient navigation, care coordination, or a similar patient-facing role. Interest or experience in obesity medicine, lifestyle change, chronic condition support. Able to efficiently use and adapt to EHRs, telehealth platforms, and a variety of digital communication and workflow tools. Strong organization skills and comfort managing multiple patient touchpoints. A strong ability to build trust, motivate patients, and communicate with empathy in a virtual setting. Health coaching certification and/or a bachelor’s degree in a health-related field preferred. A reliable internet connection and a private, HIPAA-conscious remote workspace. Schedule & Compensation Hours: 32- 4 0 hours /week , w ith flexibility based on availability and program needs. Hourly rate : $ 23 -$ 28 / hour; C ompensation is competitive and based on experience, qualifications, and overall role fit.
