Schedule: Monday – Friday 8:30 am to 5:00 pm Eastern Time Zone
We are seeking a detail-oriented and compassionate Care Management Associates (CMA) to support our newly formed Medicare Home Care Precertification team. In this role, you will work alongside nurses, medical directors, providers, and care teams to assist members with a successful transition from acute/post-acute care to the home setting.
Key Responsibilities
- Review and triage provider requests and member cases.
- Member outreach to confirm the delivery of expected home care services and adequate member support.
- Escalation of identified concerns to clinicians for follow-up and intervention.
- Communication with healthcare providers, members, and internal teams to support quality care outcomes.
- Research and resolve case-related issues while ensuring compliance with regulatory and company standards.
- Maintain accurate documentation.
- Provide administrative and customer support for operations.
Required Qualifications
- Knowledge of basic medical terminology and healthcare concepts.
- Strong customer service and communication skills, including telephone support.
- Call Center experience is required
- Excellent organizational and problem-solving abilities.
- Proficiency with Microsoft Office applications, including Word and Excel.
- Ability to work effectively in a collaborative, multidisciplinary environment.
- High School Diploma required.

The typical pay range for this role is:
$18.50 - $35.29This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
