About the Opportunity:
The CARE MANAGER role is dedicated to developing and facilitating complex care management activities for members with mental and behavioral health needs. This position ensures members receive high-quality, cost-effective healthcare through personalized care plans and education, directly impacting members and their families by coordinating necessary services and resources.
Responsibilities:
- Evaluate member needs via phone or in-home visits and recommend or facilitate appropriate care plans
- Conduct telephonic, digital, home, or site visits to assess member needs and coordinate resources
- Develop ongoing care plans for high-acuity members, identifying necessary providers and community resources
- Coordinate communication between members, families, community resources, and care providers
- Monitor care plans and member status, providing recommendations based on clinical changes
- Facilitate care coordination to ensure timely access to needed services
- Document member information and care activities to comply with regulatory requirements
- Educate members and families on procedures, treatment options, referrals, and benefits
- Provide feedback to leadership to improve care delivery and quality
- Perform other assigned duties
- Comply with all policies and standards
Requirements:
- Master's degree in Behavioral Health or Social Work, or a nursing degree from an accredited school
- 2 to 4 years of related experience
- Active RN license or Licensed Master's Behavioral Health Professional credential (LCSW, LMSW, LMFT, LMHC, or LPC) as required by state contract
- Experience with children, foster care, or juvenile justice system preferred
- Strong communication, customer service, assessment skills, and computer proficiency
- Must reside in Oklahoma County, Canadian, Cleveland, or Logan County, Oklahoma
Pay Range and Compensation Package:
- $56,200.00 to $101,000.00 per year
