Care Manager - Home Care

We are seeking a compassionate and clinically skilled Home Care Manager (RN) to oversee high-care client packages across the Perth community. This dynamic hybrid role offers the perfect mix of client-facing community visits and office-based care coordination—with no weekend work or shift work!

Key Responsibilities

• Oversee and manage complex Home Care Packages for clients living in the community.
• Conduct clinical assessments, care planning, and ongoing reviews.
• Deliver advanced clinical care, including high-care needs such as complex wound management and catheter care.
• Liaise with multidisciplinary teams, clients, and families to ensure high-quality, person-centered outcomes.
  
  

Requirements

• Bachelor of Nursing degree.
• Current, unencumbered AHPRA Registration as a Registered Nurse.
• Demonstrated clinical experience in complex nursing care (wound care, catheter management, etc.).
• Prior experience in home care, aged care, or community nursing preferred.
• Valid Australian Driver’s Licence and Rights to Work in Australia.
