Care Navigator - New York License Required

1. About Our Client:

The organization operates in the healthcare sector, focusing on remote care solutions to improve patient health outcomes. It addresses challenges in chronic disease management by supporting patients enrolled in chronic care and remote patient monitoring programs. The organization collaborates with primary and specialty physician practices and healthcare systems to deliver patient-centered care remotely.

2. About the Opportunity:

The Care Navigator role is designed to support patients with chronic illnesses by providing education and monitoring through remote care platforms. This position is essential in empowering patients to manage their health effectively while working closely with care teams. The role contributes to improving patient outcomes through consistent communication and data review.

3. Responsibilities:

• Conduct virtual consultations focusing on patient education related to lifestyle and clinical status
• Manage an assigned patient load and assist with additional patient tasks as needed
• Review and respond to remotely transmitted patient data such as blood pressure, blood glucose, weight, and pulse oximetry following approved workflows
• Document patient communications accurately
• Assist patients in setting up their remote monitoring devices
• Collaborate with team members and work independently
• Manage time and organizational tasks effectively
• Learn and apply knowledge in lifestyle medicine and clinical research
• Perform additional duties as assigned

4. Requirements:

• Valid New York Nursing License (LPN/LVN) in good standing
• Multistate/Compact Nursing License preferred
• Minimum 3 years of experience in cardiology, primary care, home care, or chronic care management preferred
• Experience in remote monitoring or virtual care management programs preferred
• HIPAA-compliant remote work environment
• Distraction-free workspace
• Reliable internet connection with minimum 100mbps download and 10mbps upload speeds

5. Pay Range and Compensation Package:

• Compensation is based on candidate''s experience and state of residence
• Opportunity to earn a quarterly performance-based bonus up to $6,000 annually
