Lead Care Coordinator @ VIM Medicine Care Coordination · Case Management • Remote (U.S.) — Eastern Time / Central Time preferred • Full-time This role works closely with clients and physicians across the East Coast and Central U.S., so we're looking for someone based in Eastern or Central time for meaningful working-hours overlap. What VIM Medicine is Health should be managed as your most important asset. VIM is building Health Management Medicine: a new clinical discipline where a physician quarterbacks your health the way a wealth manager runs your portfolio. Proactive, longitudinal, and personal. The demand for this kind of care is enormous and growing. More people are focused on their health than ever, and the options have exploded: wearables, full-body MRIs, VO2 max testing, supplements, AI advice. Navigating all of it in an evidence-based way takes a dedicated thought partner, and that's what a VIM physician is: someone managing your health with you, aimed at giving you more high-functioning years. The idea is already working. We're live in Boston with 100+ patients, delivering care with two great physicians today and two more joining this year, and we're growing faster each week. The service is resonating, and patient referrals are our top source of new members. Why this role exists We believe care coordination is where medicine's promises become real. A physician can design a brilliant plan, but whether the patient actually feels cared for comes down to what happens next. Most care coordination happens at a scale where the work can only be reactive: hundreds of patients, whoever calls loudest gets helped. We run small panels on purpose. You'll know every patient on yours: their history, their specialists, their goals, what they're worried about. That changes what the job is. You're a true partner to the physician on each patient's outcomes: in the panel reviews, tracking whether the plan is landing, catching what needs attention before anyone has to ask. 86% of our patients are meeting their health goals, and coordination done at this level is a large part of why. And you’d be doing it early: we are a startup at our core, and we are scaling rapidly, so there’s a decent amount of ambiguity in the work. It takes someone who can solve problems without a playbook and is willing to get in the weeds to build what doesn’t exist yet. What the work is You'd be the second person on our care coordination team, the number two to our Head of Care Coordination, working directly with our physicians and their patients. This is a lead role: the two of you will define what great coordination looks like and build the systems, playbooks, and eventually the team that delivers it as we grow from four physicians to many more. The craft itself is follow-through across complex patients: records, referrals, results, everything in between, closed out completely and fast. Our patients are busy people with care spread across many specialists and multiple cities, so a real share of the week is solving problems we're hitting for the first time. A stalled referral isn't a task to log, it's a problem to solve, and you keep working it until it's done. It also takes range. Our systems are early and changing, we work heavily with AI, and the right person opens an unfamiliar tool and figures it out. Our physicians are among the best in the country, and they'll rely on your skills daily. What You'll Own Own the patient journey. Be the trusted point of contact who moves each patient forward: intake and communication, scheduling across the baseline assessment and recurring visits, recaps so nothing rides on memory, and same-day responsiveness patients can count on. Close every loop. Own results and transitions of care through to confirmation: you know what happened and whether the patient understood it. You push from the inside when a specialist, ER, or hospital stalls. Move quickly while building. Keep essential clinical logistics (prescriptions, outside orders, prior authorizations) fast and reliable, and build the processes and workflows that shrink the time they take. Keep panels organized, run standing panel reviews with your physicians, and help build toward a system where every loop closes and the record is always current. Raise the standard. As the second coordinator, you'll help define how this team works: the playbooks, the escalation paths, the quality bar. You'll be a model for the coordinators who join after you. Who you are Clinically experienced. A hard requirement: direct healthcare experience in care coordination, case management, transitions of care, or clinical operations, with real comfort in patient-care workflows, orders, and prior authorizations. Thorough above all. You verify rather than assume. You have a bar for yourself and make sure every step is done. A troubleshooter. When a referral stalls, a record doesn't arrive, or a payer says no, you find the path. You escalate early, you're persistent, and you keep working the problem after most people would file it under "waiting on them." Fast and reliable to reach. Same-day responsiveness is our baseline. You over-communicate by default, and patients never have to ask twice. Genuinely tech-savvy. You learn new systems quickly rather than relying on the ones you already know. You've adopted new tools, improved a workflow, or built something that outlasted you, and you're curious about how AI changes this work rather than wary of it. Experienced at startups. You've worked at a startup or an early-stage practice before, ideally more than once. You know what it feels like when the playbook doesn't exist yet, and you bring calm and structure to it instead of waiting for someone else to. Opinionated and trusted. You have a point of view on how great care should run, you make the case for it, and you earn the trust of physicians and patients through consistent follow-through: a clinical partner, not an assistant. Discreet and precise. Sound judgment with sensitive health information, a deep commitment to privacy (HIPAA), and communication warm and exact enough to match the care we deliver. Why VIM Mission.
