About BBJ / HBCU Black Birth Justice / Healthy Beginnings Connect Us (HBCU) Inc. is a Louisville-based nonprofit organization committed to reducing racial disparities in maternal and infant health outcomes for Black and brown families. Through the HRSA-funded Healthy Start program, BBJ/HBCU provides direct services including home visiting, case management, health education, mental health support, and community outreach to pregnant and postpartum participants across Louisville Metro. Position Overview The Case Manager is an independent contractor engaged to provide comprehensive participant support services within the Healthy Beginnings Connect Us / Healthy Start program. This is a direct-service, community-facing engagement that requires a practitioner with professional social work or human services credentials, strong assessment skills, and a genuine commitment to the communities BBJ/HBCU serves. The Case Manager conducts intake screenings, develops individualized service plans, coordinates community resources, provides crisis support, and represents BBJ/HBCU in outreach settings. The contractor operates with professional autonomy and is responsible for managing their own caseload to meet all required deliverables. Scope of Services & Key Deliverables Participant Services Conduct initial screening and intake of referred participants to assess needs and determine eligibility for Healthy Start services. Deliverable: completed intake documentation submitted per participant at time of enrollment. Perform comprehensive biopsychosocial assessments identifying participant needs, strengths, risk factors, and available community resources. Deliverable: assessment documentation completed within the timeframe specified in the Contractor Services Agreement. Develop individualized service care plans in collaboration with participants, based on assessment results and participant-identified priorities. Deliverable: service plan developed per participant and updated per program protocol, minimum quarterly. Monitor and regularly evaluate participant progress against service plan goals, adjusting plans as needs change. Deliverable: updated service plan documentation submitted per protocol. Serve as a participant advocate to ensure access to services, benefits, and community resources across health, housing, childcare, legal, and social services systems. Provide crisis intervention and safety planning when participants present with mental health, domestic violence, housing instability, or child welfare concerns. Deliverable: crisis contact documentation submitted within 24 hours of each crisis contact. Conduct weekly Thursday case reviews in WFS for all open referred cases, with outreach attempts documented as they occur. Deliverable: current documentation maintained in WFS on an ongoing basis. Maintain accurate, current, and HIPAA-compliant participant records in WFS and any other designated tracking systems. Deliverable: all participant contacts, referrals, and case activity documented within 48 hours of each contact. Outreach & Community Engagement Conduct ongoing outreach to identify and enroll eligible pregnant and postpartum women within the Louisville Metro service area. Deliverable: outreach activity log submitted monthly. Provide health education and community resource information to participants and families, including information on prenatal care, postpartum wellness, infant care, family planning, and available social services. Represent BBJ/HBCU Healthy Start services at tabling events, community fairs, health fairs, and partner organization events as agreed. Deliverable: event attendance log and brief summary submitted monthly. Build and maintain professional referral relationships with community organizations, healthcare providers, and social services agencies in Louisville Metro. Reporting & Collaboration Submit all required monthly reports and caseload summaries per program reporting schedule. Attend relevant program meetings, case conferences, training sessions, and team consultations as required to fulfill contracted deliverables and maintain HRSA Healthy Start program compliance. Collaborate with Health Advocates, the Wellness Manager / Midwife, Behavioral Health Provider, and other program contractors to ensure integrated, non-duplicative participant support. Participate in HRSA-required trainings Contractor Qualifications Required Bachelor’s degree in Social Work (BSW), Human Services, Psychology, Public Health, or a closely related field Minimum two years of direct social services, case management, or community health experience serving diverse and underserved populations Demonstrated knowledge of community health principles, social determinants of health, and maternal-child health resources in Louisville Metro Experience conducting comprehensive assessments and developing individualized service plans Ability to identify and respond to crisis situations, including mental health crises, domestic violence, and child welfare concerns Strong organizational skills with demonstrated ability to maintain accurate documentation across a caseload of multiple active participants Proficiency with Google Suite (Gmail, Drive, Docs, Sheets) and experience with client-tracking or case management software Excellent interpersonal and communication skills with demonstrated ability to build trust with pregnant and postpartum participants from diverse backgrounds Culturally competent, trauma-informed practice with commitment to health equity for Black and brown families Independent, reliable personal transportation — required for field-based outreach and home visits Willingness to complete background check prior to engagement Preferred Master’s degree in Social Work (MSW) or related clinical field Current Kentucky licensure as a Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), or equivalent Registered Nurse (RN) licensure from the Kentucky Board of Nursing Prior experience with the HRSA Healthy Start program, home visiting models, or federally-funded maternal-child health programming Experience using WFS or comparable case management systems Bilingual or multilingual capability — particularly Spanish, Somali, or other languages represented in the Louisville Metro community Knowledge of Louisville Metro community resources, social services systems, and healthcare landscape
