Salary: $30.00 Hourly Summary of position The Case Manager is responsible for coordinating the person-centered Plans of Care and Service Plans for participants on-boarded by the Clinical Intake Specialist. Facilitating the selection of service providers, the Case Manager will broker services and set up authorizations based on the frequency, scope and duration of the Service Plan. The Case Manager is responsible for monitoring and confirming service delivery through monthly telephonic or in-person contact with participants and caregivers/legal guardians. Essential Functions Develop, coordinate, and implement comprehensive Plans of Care and Service Plans. Collaborate with healthcare providers, participants, and families to meet all medical, social, and personal needs. Monitor and confirm the delivery of services as outlined in the Plan of Care. Conduct monthly telephonic visits with participants or more frequently as required. Assess participants' needs, progress, and satisfaction, updating Plans of Care as necessary. Provide support and guidance to participants and families, addressing concerns. Educate participants and families about available resources and services. Maintain accurate documentation of interactions, service delivery, and care plan updates. Refer participants to relevant specialists for further coordination of benefits or transitions. Conduct ongoing care management activities, including required outreaches and condition-based updates. Screen for changes in health or conditions, ensuring care plans remain relevant. Triage and escalate cases requiring clinical expertise, reporting potential Critical Incidents. Assist with coordination and participate in interdisciplinary (IDT) meetings, discharge planning, and transitions. Prepare and submit reports as required by regulatory agencies and organizational policies. Ensure compliance with regulations, policies, and best practices in all care management activities. Position Requirements An associate degree or higher in behavioral or social sciences, or a related field, OR at least 1 year of experience in health or human services support, including interviewing individuals and assessing their personal, health, employment, social, or financial needs according to program requirements. A minimum of 2 years of experience in case management, including assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet individual human service needs. A minimum of 2 years of experience making recommendations as part of a client’s service plan, such as clinical treatment, counseling, or determining eligibility for health or human services/benefits. Experience and skills in conducting person-centered assessments and developing service plans. Skills, Knowledge & Abilities Proficient in Microsoft Word, Excel, and PowerPoint. Knowledgeable in planning principles and report preparation. Skilled in interpreting policies and procedures. Skilled in analyzing situations, evaluating information, and recommending courses of action. Ability to establish and maintain effective working relationships with colleagues and external agencies and supportive relationships with individuals and families, demonstrating competency in supporting them. Ability to evaluate issues and explain rules and regulations clearly. Capable of writing detailed reports and correspondence. Effective in presenting information and responding to questions from groups of managers and associates. Competent in defining problems, collecting data, establishing facts, and drawing valid conclusions. Ability to use automated information system to enter, update, modify, delete, retrieve, and report on data. High level of initiative, self-direction, and accountability for actions. Travel/Work Environment Remote position in Hardwick/Danville, Vermont area. Frequent travel to clients' homes or other locations as required in Caledonia and Lamoille counties. Must have and maintain a valid state driver’s license, automobile insurance coverage, and access to an automobile. Work is performed in a variety of settings, including client homes, community settings, and office environments.
