The Support Broker is responsible for coordinating the daily operation of Medicaid Waiver services, and for promoting personal growth and independence of participants in accordance with state and program regulations, GT Independence Policy and Procedures, and applicable accreditation standards. Responsibilities Support participant independence, community integration, and achievement of ISP goals. Develop and maintain relationships with participants, families, providers, and community resources. Ensure timely, accurate documentation and compliance with agency and state requirements. Monitor participant health, safety, and quality of services. Provide guidance, advocacy, problem-solving, and decision-making support. Assist with service planning, resource coordination, and access to community supports. Educate participants and families on self-directed services, budgeting, employer responsibilities, and staff management. Coordinate with healthcare providers and community organizations to support participant needs. Assist participants with housing and other independent living resources. Requirements Meet the DHCF requirements for a waiver case manager: Have current, appropriate licensure, Have a master's degree in social work, Psychology, Counseling, Rehabilitation, Nursing Gerontology or Sociology and one (1) year experience with the population(s); OR bachelor's degree and two (2) years’ experience with the population(s); OR Registered Nurse (RN); OR can have an associate degree and three (3) years’ experience with the population(s). Successfully complete a criminal background check pursuant to the District’s criminal background check, Health-Care Facility, Unlicensed Personnel Criminal Background Check Act of 1998, and the FBI Criminal History Check; Not on the DC OIG Exclusion List; Complete a required training course on the District’s Services My Way Program and traditional Medicaid program, and demonstrate an understanding of the program and the role and responsibilities of a Support Broker; Receive ongoing required training, as required by DHCF. The Support Broker may not work for the VF/EA FMS-Support Broker entity and concurrently as a waiver case manager or for a direct care service organization.
