Job Title: Case Worker II Job Description The Case Worker II provides comprehensive, culturally responsive case management services to Spanish-speaking clients and their families who are affected by HIV. This role conducts sensitive assessments, develops and coordinates individualized plans of care, and supports clients in accessing medical, psychosocial, and public benefit resources to improve health outcomes and continuity of care. Responsibilities Conduct comprehensive and sensitive assessments with clients and, when appropriate, their families or support networks to identify needs and develop individualized plans of care. Arrange, coordinate, and follow up on service referrals for clients and families, ensuring that all elements of the care plan are implemented effectively. Monitor client progress and the effectiveness of services and interventions, adjusting the plan of care as needed to address changing needs. Refer clients and families to eligible public benefits programs and assist them in understanding and accessing these resources. Maintain regular contact with enrolled clients, providing ongoing support, guidance, and advocacy throughout their care journey. Document all client contacts, assessments, service coordination activities, and follow-up in client records in accordance with program guidelines and documentation standards. Educate clients and families about the basic medical and psychosocial aspects of HIV and related conditions to promote understanding, engagement in care, and adherence to treatment. Provide proactive follow-up to track client symptoms, treatment responses, and engagement in care, intervening early to address barriers or emerging needs. Adhere to all applicable state laws, regulations, and departmental policies governing program operations and client services. Stay current with new resources, programs, and developments relevant to HIV care and social services, integrating this knowledge into client support. Participate actively in continuous training and professional development opportunities, including staff meetings and assigned community meetings. Collaborate with interdisciplinary team members and community partners to coordinate care and ensure a holistic, client-centered approach. Essential Skills Fluency in Spanish and English, with the ability to communicate effectively and sensitively with Spanish-speaking clients and families. Bachelor’s degree in Social Work with at least 3–5 years of relevant experience, or a master’s degree in a related field (master’s preferred). Experience providing social work or case management services, including assessment, care planning, and service coordination. Strong interpersonal and communication skills, with the ability to build trust and rapport with clients facing complex medical and psychosocial challenges. Ability to conduct comprehensive, sensitive assessments and develop individualized plans of care. Skill in coordinating referrals and navigating community resources, public benefits, and support services. Proficiency in maintaining accurate, timely, and compliant documentation in client records according to program guidelines. Knowledge of basic medical and psychosocial aspects of HIV and related conditions, or the ability to learn and apply this knowledge quickly. Basic Life Support (BLS) certification. Ability to adhere to state laws, regulations, and organizational policies related to client care and program operations. Commitment to ongoing learning, professional development, and participation in training and meetings. Additional Skills & Qualifications Master’s degree in Social Work or a related field is preferred. Previous experience working with individuals affected by HIV or other chronic health conditions. Experience serving Spanish-speaking communities and understanding of cultural factors that may influence health behaviors and engagement in care. Strong organizational and time management skills, with the ability to manage multiple clients, tasks, and deadlines. Ability to work collaboratively within an interdisciplinary team and with community partners. Comfort with discussing sensitive topics, including health status, treatment adherence, and psychosocial challenges. Flexibility and adaptability in a dynamic environment that requires ongoing learning and responsiveness to client needs. Work Environment This position supports Spanish-speaking individuals who are affected by HIV, including those who are newly diagnosed, have been lost to care, are not consistently engaged in treatment, or are experiencing viral resistance and require intensive support. The role follows a Monday–Friday schedule from 8:00 a.m. to 5:00 p.m. The position is primarily onsite for four days per week, with one day per week available for remote work, and the specific remote day can be selected in coordination with the team. The work environment involves close collaboration with healthcare providers, social service agencies, and community organizations, and requires frequent client interaction in both office and community settings. The focus is on providing a supportive, respectful, and client-centered atmosphere while using standard office technology and documentation systems to manage client information and care coordination. Job Type & Location This is a Contract to Hire position based out of Saint Louis, MO. Pay and Benefits The pay range for this position is $28.00 - $29.00/hr.
