Clinical Care Coordinator / Patient Advocate
Location: Remote – South Africa
Working Hours: Monday to Friday, 3pm to 12pm (40 weekly hours)
Employment Type: Full-Time

About the Role
Our US-based healthcare client is seeking an experienced Clinical Care Coordinator / Patient Advocate to support patients within a skilled care facility and help ensure they receive the appropriate level of care and treatment.
This role is ideal for a healthcare professional with strong clinical knowledge and experience working within a large healthcare or facility-based environment, who understands the patient journey and can confidently advocate for continued treatment when a patient may be at risk of being discharged before they are ready.
The successful candidate will work closely with nurses, physicians, Physical Therapists (PTs), Occupational Therapists (OTs), clinicians, patients and families to understand patient needs, communicate the value of continued treatment, and support appropriate care decisions.
Key Responsibilities
- Advocate for patients to ensure their clinical needs and best interests are understood and appropriately addressed.
- Review and understand patient treatment plans, progress and ongoing care requirements.
- Help communicate the clinical benefits of continued treatment within a skilled care facility.
- Engage with patients and families to explain treatment plans and encourage participation in recommended care.
- Work collaboratively with nurses, PTs, OTs, physicians and other clinical professionals.
- Understand rehabilitation and skilled-care processes and recognize when a patient may continue to benefit from treatment.
- Assist in communicating and advocating for appropriate continued care when a medical plan or insurer is recommending discharge.
- Review relevant clinical information and documentation to support patient-care discussions.
- Build strong relationships with patients while demonstrating empathy, confidence and excellent communication skills.
- Coordinate with internal clinical teams to ensure patient needs are clearly communicated.
- Maintain accurate and professional documentation relating to patient interactions and care coordination.
- Follow established healthcare policies, procedures and confidentiality requirements.
Requirements
- Clinical or healthcare background is essential.
- Previous experience working in a hospital, skilled nursing facility, rehabilitation facility, long-term care facility or other large healthcare setting.
- Strong understanding of clinical processes, patient care and rehabilitation.
- Experience working alongside or closely with nurses, Physical Therapists, Occupational Therapists, physicians and other clinicians.
- Strong patient advocacy skills with the ability to confidently communicate why continued care may be beneficial.
- Excellent verbal and written communication skills.
- Ability to explain complex medical or clinical information in a clear and compassionate manner.
- Strong interpersonal skills and the ability to build trust with patients and healthcare professionals.
- Confident, persuasive and empathetic communication style.
- Strong administrative, organizational and documentation skills.
- Ability to work independently in a remote environment while collaborating effectively with a US-based healthcare team.
- US healthcare experience is NOT required. Candidates with strong South African healthcare experience and clinical knowledge will be considered.
