Clinical Caseload Management: Manage a moderate caseload (~125-150 cases, 20-30 of which are more complex cases) requiring coordinated referrals, clinical triage, scheduling, and follow-up. Independent Navigation: Manage assigned patient caseloads and coordination tasks, balancing clinical advocacy with engagement goals. Documentation & Workflow: Ensure accurate clinical documentation, opportunity tracking, and follow-through across care workflows. Cross-Functional Alignment: Coordinate with external providers and internal partners to support seamless care delivery and optimal patient adoption. Continuous Improvement: Identify recurring operational issues, objection patterns, or engagement barriers, surfacing actionable improvement opportunities. Mentorship: Support onboarding and ramp-up of newer team members in best practices for both patient care and consultative communication. Compliance: Maintain strict HIPAA compliance, clinical standards, and data integrity standards. Qualifications and Requirements: Active Registered Nurse (RN) License required (unrestricted state license; compact license preferred). Bachelor of Science in Nursing (BSN) required. Experience in Oncology nursing or dedicated oncology navigation required. 2+ years of experience in a consultative, sales, account management, or patient-conversion role (e.g., healthcare sales, patient intake/recruitment, health coaching, or clinical advocacy) preferred.
