Clinical Case Manager – Behavioral Health (Licensed)
100% Remote | Anywhere in the U.S. | Monday–Friday Business Hours
No Weekends • No Holidays
Are you a licensed behavioral health clinician who thrives on helping members navigate complex mental health and substance use challenges? Join our team and use your clinical expertise to provide comprehensive case management, crisis intervention, and care coordination that improves outcomes and supports whole-person health.
What You'll Do
- Conduct comprehensive behavioral health assessments using clinical tools, data analysis, and benefit plan reviews
- Develop individualized care plans that address both behavioral and physical health needs
- Coordinate care with providers, community organizations, and support services
- Apply advanced clinical judgment to identify and mitigate risk factors impacting member outcomes
- Perform crisis intervention for members experiencing behavioral health or medical crises and ensure appropriate referrals and follow-up
- Utilize Motivational Interviewing techniques to engage members and promote sustainable behavior change
- Coordinate psychosocial wraparound services and connect members with community resources
- Support cost-effective care through proactive care planning and resource utilization
WHAT YOU BRING – REQUIRED
- Master's degree in Behavioral Health, Social Work, Counseling, Psychology, or a related field
- Active, unrestricted independent behavioral health license in your state of residence (e.g., LCSW, LISW, LMFT, LPC, LCPC, LPCC, LMHC, or equivalent)
- 3+ years of post-master's direct clinical experience in a hospital, ambulatory, outpatient, or similar clinical setting
- 3+ years of experience treating mental health and substance use disorders
- 3+ years of experience utilizing Motivational Interviewing techniques
- Case Management experience
- Ability to simultaneously talk on the phone and document in clinical systems
- Strong technical proficiency and comfort working in a virtual environment
Preferred Qualifications
- Crisis intervention experience
- Managed Care experience
- Utilization Review or Utilization Management experience
- Discharge Planning experience
- Experience working with complex behavioral health populations

The typical pay range for this role is:
$60,522.00 - $129,615.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
